Record Request and Private School Application Process

Private School or Program Application Process

Please review all of the required forms below as all forms must be submitted to @email or handed in person to Student Services Registrar a minimum of 10 school days before the due date. Please plan accordingly as school holidays/winter break do not count towards school days.

  1. Complete the "Private School Request or Program Application Information Form"  and the "Parental Consent Form" and email it to [email protected] or hand in person to Student Services Registrar.
  2. Transcript Fee: The first requested copy of the transcript is free; each additional transcript request is $5 per transcript. Payment is accepted through MySchoolBucks only. Kilmer Middle School will not email the additional transcripts until payment has been received. 

If teacher or counselor letter of recommendation is required after requesting the recommendation in person, email the: “Request for a Letter of Recommendation Form” directly to the teacher or counselor.

**Please submit the "Private School or Program Application Information Form" & the "Parental Consent Form" form to [email protected] or hand in person, PRIOR to requesting a recommendation from your teacher.**

Request for Academic Records

If you would like to request a copy of your child’s records, please complete the Parental Consent Form and email it to @email or hand in person to Student Services Registrar. There is a 5-day turnaround time.
